JustOT are recruiting for an experienced HCPC registered Occupational Therapist, to work with our well-respected client based in Somerset.
We are looking for an Occupational Therapist to provide statutory occupational therapy and care planning services to vulnerable members of the community , focusing on physical function and supporting equipment, to reduce the impact of ill health and disability, and improve independence, outcomes and quality of life for individuals.
The role is a fully qualified Occupational Therapist with their own varied caseload, receiving professional supervision and guidance, working in multi-agency and inter-professional settings.
Undertake assessments of need and risk in relation to physical function and support needs, to inform decisions and care planning that will support vulnerable individuals.
Take professional, evidence-based, risk-informed decisions in line with legislation, statutory guidance, and local policies and procedures, to ensure appropriate action is taken.
Create and deliver strengths-based, outcome focused support plans and arrange equipment provision to deliver positive outcomes and promote greater independence for individuals, families and carers.
Assess need, specify and demonstrate equipment/aids, adaptations and assistive technology to support individuals' physical and mobility needs and promote independence. Includes arranging equipment provision and liaising extensively with housing providers.
Review support plans regularly to evaluate progress and ensure that it is meeting assessed needs and delivering improvement for service users, making changes and adjustments to support plans as required.
Identify and monitor risk, addressing risks where possible and escalating as required, to ensure that situations are addressed at the earliest possible opportunity and risk minimised.
Build collaborative, professional relationships with service users, taking a person-centred approach to ensure that service users are fully enabled to participate in planning and decision making regarding functional support and their own lives.
Manage a caseload (including complex and high-risk cases) where service users have diverse and cumulative requirements.
Work collaboratively with other professional teams and over organisational boundaries, ensure service users experience cohesive and seamless support to meet their needs.
Maintain accurate professional records and case files in accordance with statutory requirements and local policies and procedures, to ensure that information is auditable.
Hourly rate - £31.28 ph
Location: Bath
Hours & Days to be Worked: full time , Mon to Friday
Working arrangements : Mix of home working and office
Essential Requirements : HCPC registered , 2 years Social Services Experience , Car driver
