JustOT are recruiting for an HCPC registered experienced Occupational Therapist , to work with our well-respected client based in Central West London.

We are looking for an Occupational Therapist to Join our Adult social care team -

Occupational Therapy Care Manager (OTCM) to join the Community Independence Service (CIS) Reablement Team.
This is a frontline role supporting residents to regain independence following hospital discharge or health crises. The successful candidate will work as part of a multidisciplinary team, including Occupational Therapists, Independent Living Assessors (ILAs), and Social Workers, and will collaborate with health professionals and Reablement care providers.
Key Responsibilities: Deliver skilled Occupational Therapy interventions to residents in their homes.
Manage cases for up to six weeks with a goal-focused reablement plan.
Conduct holistic assessments to identify clinical and personal needs.
Develop and implement intervention plans, including complex movement and handling assessments.
Order and provide adaptive equipment to support independent living. Work collaboratively with a multidisciplinary team to ensure effective service delivery. Maintain accurate case records using the Mosaic system. Essential Criteria: HCPC-registered Occupational Therapist. Experience in adult social care or community settings. Experience in complex movement and handling assessments and care planning (training and supervision available). Strong case management skills, with experience in goal setting and strength-based interventions. Ability to work effectively in a fast-paced, multidisciplinary team. IT proficiency, with experience in Mosaic preferred.

Hourly rate :£33.00
Location: West London
Hours & Days to be Worked: full time , Mon to Friday
Working arrangements : Mix of home working and office
Essential Requirements : HCPC registered , 3 years Social Services Experience , Car driver
